Outpattle chemotherapy, altamonte springs, fla.–based adventhalth cancer institute attained within 30 days after crossing the benchmark rate of CMS for the emergency room of cancer patients and the benchmark rate of the hospital, eight strategies were identified and applied to improve outpatient cancer care.
The strategies helped the system reduce its 30-day reduction rate by 13.5%, which is less than the 10.4% benchmark prescribed by CMS, from 8.4% internal measurement of 8.4%, according to an essence presented in the Oncology Nursing Society Congress on 10 April.
There were eight strategic intervention:
- Offering guidance to improve the patient’s education and to pay attention to medical attention on common side effects of chemotherapy.
- Installation of an oncology nurse hotline, providing immediate advice and access to patients.
- To discuss the symptoms and to offer guidance to deliberately conduct follow -ups with high -risk population in three to five days after each treatment.
- Addressing the patient’s social and medical requirements by attaching a multi -check care team.
- Developing a triage pathway and using other care team members to reduce the burden on the provider schedule.
- Determining the infusion appointments of the same day for outpatient symptoms management.
- Partnership with a home-based immediate care service.
- Establishing a reporting structure that allows for regular monitoring of ED visits and reduction matrix, including continuous refinement of care processes based on data and response from patients and employees.
