The Henry Ford Health System at Detroit is ready to offer hospital-level care at home settings, thanks to a new partnership with Costa.
The partnership will offer several domestic health services, including acute care services, efficient nursing care and subcutaneous care.
Under the hospital at home program, eligible patients can choose to get the same care in the hospital in your home through a combination of virtual and in-in-Persen services. The same is true for Henry Ford’s efficient nursing care at home and subscribing care in home programs, providing additional emotional support for people with serious illnesses.
The health system is first rolling the program at Henry Ford Hospital in December and as per the announcement, other Henry Ford is planning to expand to the hospital locations.
Contessa has created a name for itself to provide home healthcare through its care convergence platform that claims risk-based-model and claim analysis capabilities. The company is already partnership with several health systems, and the Mount Sinai Health System, Commonpirit Health, Marshfield Clinic Health System and Payers.
The care model of Contessa has a 44% decrease in redmission, 35% decrease in living length and 90% patient satisfaction score.

Why does it matter
While hospitals are historically the primary place of acute care services, new models of care are triggering a paradigm change.
