The US is the largest donor for global health and has given priority to the health to a greater extent than other donors governments within its foreign aid activities. Apart from being the largest donor, the US operates, provides technical support, participates in international health organizations, does research, and more. Since the beginning of his second term, President Trump has taken several action affecting American global health efforts, including foreign aid reviewing existing programs, the end of the US Agency for International Development (USAID), which manages the majority of American global health funding, and signs several functioning tasks that ban American global health activities. These attempts have significant implications for existing American programs and the donor has abandoned large intervals in the landscape.
Recently, as part of its foreign aid review, the Trump administration informed the Congress about the number of USAID Awards, which he intends to finish and continue it. The list included over 6,200 awards, of which more than 5,300 were abolished and around 900 remaining active as active. These awards include $ 27.7 billion in funding of more than $ 36.0 billion in total and more than $ 8.3 billion in Terminated Awards. Although it is unknown whether it would be the final list, it provided the first official indication of administration schemes for US foreign aid portfolio for global health activities. Recently KFF analysis examined some questions raised by the list. This analysis provides assessment of the number of active and finished global health project awards (detailed methods are given below).

The Global Health Awards accounts for 12% of all the awards, but all uncontrolled money majority (53%). Of over 6,200 awards in the list, 770 (12%) are eligible as global health awards. The awards represent an estimated $ 19.1 billion, which is uncontrolled funding (defined by the total prize price and already disrupted amount by administration), or 53% of all uncontrolled funds.
80% of the Global Health Awards have been abolished, which is a total amount of $ 12.7 billion in total. Of the 770 Global Health Awards identified, 615 (80%) has been abolished, slightly below all the awards (86%). These finished prizes include support for polio vaccination programs, activities aimed at improving maternal and child health (MCH), efforts to strengthen infectious diseases around the world, HIV treatment projects, family planning assistance, and more. Many global health awards are multi-regional, which extend more than one global health sector, with HIV/AIDS accounting for the largest number. Of the 770 awards identified, 289 specify activities that address more than one of the six main global health sub-areas, including: HIV/AIDS; Tuberculosis (TB); Malaria; Maternal and Child Health (MCH); Family planning/reproductive health (FPRH); And nutrition. The award which included HIV/AIDS activities was responsible for the largest number (379), followed by MCH (266), and FP/Rh (233) (see Figure 1).
By sub-region’s activity, the boundary of awards was eliminated between 71% for HIV and 86% for MCH and 86% for NutritionMost of the awards for each sub-sector activity were abolished. MCH and Nutrition had the largest part of the awards ended (86%for both sub-sectors), followed by FPRH (85%), malaria (80%), tuberculosis (79%), and HIV (71%) (see Figure 2).

| To assess which awards were health-related in the Deduplicated Usaid list, we earlier tried to match the award ID for all awards (6,232) in the list with activity project number in the database of all foreign aid funding. The database includes each of these (eg, HIV, TB, malaria) for awards (eg, health, education, agriculture, etc.) and sub-sector, which allow official coding of data for any matching award. Among the total, 3,129 awards were received. What we did not match for 3,103, we previously excluded any award in the list released by USAID’s Bureau for Humanitarian Assistance (BHA) and Bureau for Democracy, Conflict and Humanitarian Assistance (Dcha), excluded any award on the list released by other offices, which is considered to be a humanitarian aid award, but not all offices, but not all offices. (GH). Of these, we reviewed their reward text details to determine whether the award includes identifying health activities and if HIV, tuberculosis (TB), malaria, maternal and child health (MCH), nutrition and family planning and family planning and reproduction and reproduction health (FPRH) were known to identify sub-region activities. An award which included HIV/TB activities was coded as HIV. We did not include awards for water, hygiene and sanitation (wash) as health as these awards may include infrastructure projects, water management programs and other activities that are not included under American global health programs. While an award detail included activities in more than one sub-sector (either in our hand-coding), all these sub-sectors were counted, resulting in a large number of prize activities compared to the total number of health awards by the Health Sub-region. Our approach is likely to have a total number of global health awards (and allied funding) due to the limited nature of the information included in the details for several awards that do not match with the data. For example, there were several awards for the Ocean Freight Contracts, which did not explain what was being sent, which could include items for global health programs, as well as many awards to list individual advisory names or construction projects, some of which may be healthy. Our analysis of health sub-sectors is also limited that it is possible that for those spread over one area, only a part of the award was active (or finished), and our approach would include all sub-regions. Finally, if an award was listed twice, each with the same position (both were active or listed as termination), one of the duplicate lines was removed (an example for global health); If an award was listed twice, each with a separate position (an active and a finished), both were removed from the list (an example for global health). |
